Privacy Policy

Hospice Prince Edward Foundation and Hospice Prince Edward (collectively, “Hospice Prince Edward”) are committed to protecting the privacy of the personal information of its employees, volunteers, members, clients, donors and stakeholders.

We value the trust of those with whom we deal, and of the public, and recognize that maintaining this trust requires that we safeguard the personal information you choose to share with us. We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the services we provide.  

Hospice Prince Edward provides a broad range of health services and supports to clients in residence and in community-based service programs. To meet clients’ needs and provide effective care and service, Hospice Prince Edward needs to collect, use and disclose (only where necessary to medical professionals) personal health information (“PHI”). As such, Hospice Prince Edward is the health information custodian, as defined in the Personal Health Information Protection Act 2004 (“PHIPA”) and is accountable for the protection of clients’ personal health information. All other fundraising, donor and communications activities involving personal information are kept in compliance with the Personal Information Protection and Electronic Documents Act (“PIPEDA”).

Definition of Personal Information

Hospice Prince Edward collects personal information about an identifiable individual such as an individual’s name, home address, email address, telephone number, age and dob (“PI”).  Business contact information such as names, titles, business addresses, telephone numbers or business email address of an employee or those of which are publicly available or information that cannot be associated with a specific individual, such as information that is aggregated or are anonymous are not considered personal information.

Privacy Practices

PI gathered by our organization is kept confidentially. Hospice Prince Edward collects and uses PI only for the reason(s) for which it was provided. Safeguards are in place to ensure that the PI is disclosed only as necessary and required to achieve the purpose for which it was gathered and in accordance with our privacy policy. We take measures to ensure that the integrity and accuracy of the PI collected is maintained and to prevent misuse, inadvertent disclosure, sharing or destruction.  In this regard, staff, volunteers, external consultants and others associated with Hospice Prince Edward are bound by Confidentiality Agreements. It is the policy of Hospice Prince Edward not to sell or share information about its client’s (or clients’ POA or family members), members, donors, or volunteers with third parties.

Types of Information Collected:

PHI: The purpose of collecting personal health information is to provide health services to clients. In all but exceptional circumstances, Hospice Prince Edward uses consent to collect, modify, use personal health information. In general, the information collected consists of:

  • Client’s name

  • Date of Birth

  • Diagnosis

  • Weight

  • Ontario Health Card number

  • Health care history and health facts

PI: Is collected through face-to-face meetings with Hospice Prince Edward staff, over the telephone, and through written or electronic documents. Information is collected at an initial meeting and/or through initial submission of admission applications, over time as care and service is provided and when follow-up is completed. The documentation of personal health information by Hospice Prince Edward on paper and in electronic format must comply with internal standards, policies and procedures related to confidentiality, the security of information and professional practice standards.

Donor Information: All information concerning donors or prospective donors, including their names, addresses and telephone numbers, the name of their beneficiaries, the amount of their gift, etc., shall be kept strictly confidential by Hospice Prince Edward, its staff and volunteers, unless consent is obtained to share from donors to release or share such information with a third party. Information collected is used to keep donors and community informed and encourage continued support for upcoming events, fundraising opportunities and campaigns that supports the work of Hospice Prince Edward. Hospice Prince Edward is committed to respecting the privacy of its donors. 

The types of donor information that Hospice Prince Edward collects is:

  • Donor name, address, telephone number and email address

  • Donor giving information such as: events attended, publications received and special requests for program information

  • Information provided by the donor in the form of comments and suggestions

Limiting Use, Disclosure and Retention

We will use and disclose client PHI collected by Hospice Prince Edward, its staff and volunteers and donors PI only for the purposes we have identified above and only except as otherwise permitted or required by law.  We retain PHI and PI only for as long as reasonably required to serve those purposes. 

IP Address & Unique Identifier Information

Hospice Prince Edward may collect and track Internet Protocol (IP) addresses and other unique identifiers every time a user visits our website. Our website has cookies that inform us, at an aggregated level how our website is being used, how many people have visited and what pages were viewed.  This information is used by Hospice Prince Edward to track site traffic for statistical analysis and internal reporting purposes.

Surveys

At times, Hospice Prince Edward may conduct surveys of its members, donors, or volunteers. Should you choose to participate in such surveys, Hospice Prince Edward will use the information you provide to better understand your needs as a member, donor or volunteer of Hospice Prince Edward.

Updating of Privacy Policy

Hospice Prince Edward will review its Privacy Policy and privacy practices periodically and reserves the right to change this Privacy Policy at any time and from time to time. Please check our website for information on our most up to date policy.
