Service Coordinator

Position summary

Reporting to the Executive Director (ED) of Hospice Prince Edward (HPE), the service coordinator is responsible for coordinating, administering, documenting and maintaining an effective and comprehensive Hospice Community Visiting & Bereavement Program. He/she will be working closely with clients and their families in homes, long-term care facilities, hospice and hospital environments, assisting individuals and families to optimize their end of life journey through our volunteer services. The Service Coordinator is responsible for recruiting, training, screening, supervising and mentoring of all volunteers of HPE as well as facilitating educational opportunities.

Qualifications

  • A degree or diploma in social services, nursing, or a proven combination of complimentary education and experience.
  • Knowledge and experience related to community-based end-of-life care and journey involved for client and family.
  • Experience working with Volunteers and diverse groups of people.
  • Knowledge of Hospice services, health care and relevant community resources in Prince Edward County.
  • A sound working knowledge of related community and Provincial/Federal resources.
  • An understanding of and experience with the grieving journey.
  • Computer skills in MS Word, Excel and client management systems.
  • A valid driver’s licence and the use of reliable vehicle.

Duties and Responsibilities - Volunteers:

  • Implement and evaluate all volunteer activities including recruitment, training, orientation, support services and continuing educational activities.
  • Recruit, screen, interview and maintain a core group of volunteers.
  • Implement and evaluate basic training and other volunteer educational activities including jointly organized with other agencies.
  • Ensure volunteer satisfaction.
  • Recruit committee volunteers when necessary.
  • Ensure accurate records.
  • Coordinate volunteer’s shift assignments.
  • Provide non-clinical volunteer supervision and assignment of duties.
  • Maintain volunteer lists, statistics and forms.
  • Participate in program development and quality improvement initiatives.

Duties and Responsibilities - Clients:

  • Manage and monitor all client services including client assessments.
  • Plans, implements and administers the Visiting Hospice & Bereavement Programs at HPE.
  • Ensures the program provides services based upon clients’/caregivers’ needs within the parameters of policies and procedures established by HPE.
  • In conjunction with clients and caregivers, develops and implements appropriate individual program plans.
  • Match the appropriate volunteers with clients who request volunteer assistance.
  • Develops and implements an established method for informing the public and other service providers of program services.
  • Works collaboratively with other agencies/individuals while supporting clients.
  • Manage and monitor all Bereavement Client referrals including intake and assessments and development of a care plan as required, including planning and/or facilitating peer support groups.
  • Ensure that policies and procedures regarding the delivery of Clients services comply with HPCO Accreditation timelines.
  • Serves as a resource for the County regarding bereavement needs and support.
  • Information Management:

- Collects a range of personal information from clients and volunteers, and follows protocols for confidentiality.
- Develops and maintains current, accurate volunteer lists as well as program statistics regarding client services.
- Develops and maintains administrative systems for the effective management of volunteer information.
- Maintains individual volunteer and client files and ensures that information is always current, accurate and complete.
- Accurately summarizes and notates relevant information, taking care to ensure objectivity and absence of interference.

Duties and Responsibilities - General:

  • Maintains standards as set out by HPCO Accreditation Standards.
  • Fosters a work environment that promotes collaboration and positive regard for clients, families, volunteers and staff.
  • Participate in program development and quality improvement initiatives.
  • Participate in regular staff meetings.
  • Coaches and mentors students as appropriate.
  • Participates and assists in fundraising activities and other Hospice functions.
  • Contribute to and assist in composing the Newsletter and other relevant correspondence.

These duties are reflective of the position, but not necessarily all inclusive; other duties may be assigned by the Executive Director in consultation with the Service Coordinator.
